Forms & Requirements
IMMUNIZATION REQUIREMENTS:
All college students in New York State are now required by law to document their immunity against measles, mumps and rubella. If you were born on or after January 1, 1957, you cannot be enrolled in your degree program unless you comply with the requirements before you register.

Please have your physician or other health care provider review the attached fact sheet and complete the appropriate medical form. Bring the signed, original form to orientation, or send it to:

INSURANCE INFORMATION:
Although seminarian insurance rates are broken into monthly amounts, students receive health insurance invoices three times per year. The fall semester invoice covers a four month period (September through December). The spring semester invoice covers a five month period (January through May). The summer invoice covers a three month period (June through August).
The Episcopal Medical Trust offers four different seminarian health plans. The rate schedule for each plan is based on the complexity and features of the plan, the age of the seminarian, and the plan tier (single, plus one, family, etc).
