Forms & Requirements

IMMUNIZATION REQUIREMENTS:

All college students in New York State are now required by law to document their immunity against measles, mumps and rubella. If you were born on or after January 1, 1957, you cannot be enrolled in your degree program unless you comply with the requirements before you register.

STUDENT HEALTH INSURANCE INFORMATION:

All full-time students are required to have health insurance coverage. Coverage can be obtained through an outside vendor or through vendors provided by GTS. GTS offers you and your family health and dental insurance with The Episcopal Church Medical Trust administered through the Church Pension Group. The Episcopal Medical Trust offers three different seminarian health plans. The rate schedule for each plan is based on the complexity and features of the plan, the age of the seminarian, and the plan tier (single, plus one, family, etc).

Health Plans Offered

  • Aetna Health Fund Plan
  • Aetna HMO Plan
  • Empire BCBS PPO Plan

Although seminarian insurance rates are broken into monthly amounts, students receive health insurance invoices three times per year. The fall semester invoice covers a four month period (September through December). The spring semester invoice covers a five month period (January through May). The summer invoice covers a three month period (June through August).

During Orientation, a representative from The Church Pension Group will explain the various plans.  Registration Forms will be available.
